Reportable Incidents
Reportable Incident Forms are kept in the folder marked as such. Read the back of the form for the definitions of what is reportable. If a reportable incident has occurred a form must be completed and Licensing is notified within 24 hours.
The form will be completed by the caregiver who was most directly involved in the incident. Instructions on how to fill out the form are also in the file folder. The Manager/Licensee will review and sign the form.
The white copy is kept for the facility and is placed in the child’s file. The yellow copies are mailed to Licensing at Health Protection-Community Care Licensing Facilities Suite 300-205 Newport Drive, Port Moody, BC V3H 5C9. The Manager will phone Licensing within 24 hours of the incident at 604-949-7700. The pink copy goes to the funding agency, if applicable.

Steps on how to handle incident reports
Here are steps to follow to report an incident.
1. FILL OUT THE INCIDENT REPORT FORM:
(Specific forms are provided by Community Care Facilities Licensing. If you are in need of this document call your local Licensing Office).
- Give all details of the incident include the date, time, people involved, and explanation of the incident and witnesses accounts (supervisors, assistants, and third party accounts).
- Explain in the report the new procedure staff will follow in order for the incident not to happen again.
You must report to Licensing by telephone within 24 hours of the incident.

4. FOLLOW UP WITH A PHONE CALL to the children’s parent that was involved in the incident. Manager should make this call. Assistants can make this call only if the manager has directed and consulted with the Assistant.
Reportable Incidents
(Taken from the BC Licensing Regulation)
- Aggressive or Unusual Behaviour: by a person in care towards other persons, including another person in care, which has not been appropriately assessed in the individual’s care plan.
- Choking: a choking incident involving a person in care that requires emergency care by a medical practitioner, or transfer to a hospital.
- Emergency Restraint: any use of a restraint that is not approved and documented in the care plan of a person in care.
- Emotional Abuse: any act, or lack of action, which may diminish the sense of well being of a person in care, perpetrated by a person not in care, such as verbal harassment, yelling or confinement.
- Fall: of seriousness as to require emergency care by a physician or transfer to hospital.
- Food poisoning: a food borne illness involving a person in care that requires emergency care by a medical practitioner or nurse practitioner, or transfer to hospital.
- Medication Error: an error in giving medication to a person in care and requires emergency intervention or transfer to hospital.
- Missing or Wandering Person: a person in care who is missing.
- Neglect: failure of care by the care provider, including food, shelter and supervision.
- Physical Abuse: any physical force that is excessive for, or is appropriate to, a situation involving a person in care and perpetrated by a person not in care.
- Poisoning: ingestion of a poison or toxic substance by a person in care.
- Sexual Abuse: any sexual behaviour directed towards a person in care by an employee of the licensee, a volunteer or any other person in a position of trust, power or authority and includes:
b) Sexual activity that the older or more powerful person in care is clearly taking sexual advantage of the younger or less powerful person in care.
- Unexpected Illness: of seriousness that is required a person in care to receive emergency care by a physician or transfer to hospital.
